Your Privacy

At the Kirkland and District Hospital, we recognize that the health information of our patients deserves to be treated with respect and sensitivity and our patients’ privacy must be protected. Access to your personal health information is available to those who need to know to provide care, including: physicians, nurses, technicians, therapists and other health professionals. Furthermore, when it comes to collecting and using your personal health information, we take measures to ensure the privacy and confidentiality of the information is protected.

KDH is committed to protecting the privacy of personal information under the custody or control of KDH in accordance with the following acts:

Personal Health Information Protection Act (PHIPA)
Freedom of Information and Protection of Privacy Act (FIPPA)
Quality of Care Information Protection Act (QCIPA)

Kirkland and District Hospital collects, uses, discloses, and retains personal health for the purposes of:

Direct patient care;

  • Administration and management of the health care system;
  • Quality Improvement (such as satisfaction surveys) and risk management activities;
  • Research, teaching, and compiling statistics;
  • Fundraising (name and address only);
  • Complying with legal and regulatory requirements.

Your personal health information will only be used in compliance with Kirkland and Districts Hospital’s Privacy Policy (see attachment below). If you wish to learn more about how your personal health information will be used and the security measures being taken to safeguard your privacy, please ask your health care provider. Your health care provider may refer you to the Privacy Coordinator for more information (see attachment below).

Your request for health care services implies consent for the handling of your personal health information for specific, related purposes. Unless a new purpose is required by law, your personal health information will not be used for a new purpose without your consent.

You may withdraw consent in writing at any time, subject to legal or contractual restrictions and reasonable notice.

Kirkland and Districts Hospital has guidelines and procedures with respect to the retention and destruction of personal health information, the details of which are available on request.

Upon request, you have a right of access to, and correction of, your own personal health information. Kirkland and Districts Hospital can also provide an account of the use that has been made, or is being made, of this information and an account of third parties to which it has been disclosed.
